CDA Event/ Meeting Request Form

This form is to be filled out by department head leaders to request any type of event/ gathering at or on behalf of CDA. Please make sure to submit this form at least 60 days prior to the event or 30 days prior to a meeting for approval. Once approved by the pastoral office you may submit the requests on Planning Center Resources and/ or submit a communications request form if necessary.

If this is a multi day event please mention the specific dates and times in the comments section.

If the event/ meeting requires a budget then please have separate conversation about this with Mauricio.

Ex: Email, Announcements, Social Media, Flyer, Slides/ Presentation, Video

Please list all the names of anyone (including yourself) that will be addressing the audience.

Is there a follow-up with the attendees? Invitation to church? Next meeting or event?
