This form is to be filled out by department head leaders to request any type of media such as Emails, Announcements, Social Media, Printed Materials, Shirts, or any Communications for Casa de Alabanza. Please submit this form 60 days prior to your due date.

*If you will be creating your own art, media, or communications it must be first approved by the church communications team. Please fill out this form and state in the comments that you are proposing your own work. Please make sure to email your proposed work to JD@cdamiami.org*
